FY 2008 Audit Instructions


Preface

Welcome to AUDIT08!  If you are already familiar with the IHS Diabetes Chart Audit process, read the information below for a brief summary of this year's changes followed on the next page by quick-start directions. More detailed instructions for any of the steps are available in subsequent sections.

If you have not previously participated in the annual Diabetes Audit, please take time to read carefully through Sections 1-9 before beginning your audit activities.

What's new in AUDIT08?

New items

There is a new element concerning documentation of an estimated glomerular filtration rate (eGFR) during the audit period.

Deleted items

The second-most-recent HbA1c value is no longer required.  Only a single value, the most recent HbA1c, is now requested.

Modification of items/reports

The DM Therapy section contains additional choices: Incretin mimetics, DPP4 inhibitors.

  

A Note on Epi Info

Epi Info is available in DOS (version 6.04d) and Windows (version 3.3.2).  The AUDIT07 files will be available for the DOS version of Epi Info.  A number of challenges have kept us from adopting the Windows version (one being that many email programs refuse to attach or transmit its .mdb files, as they consider them potential security risks).  We are currently working on a web-based data entry system that will avoid such problems. You may wish to periodically check here for updates on our progress.

 

Quick Start Directions for the manual diabetes audit:

1. Check with your Area Diabetes Consultant to see if an Area-wide Local Option Question has been developed for your Area.
     If a local option question will be used, print it onto the audit form.

2. Select in random fashion the appropriate number of charts to review.

3. Review the 2007  audit form, definitions and criteria with all chart reviewers.

4. Perform the chart reviews and check to be sure the audit form is completely filled in, particularly all examinations imunizations, and education.

5. Download the AUDIT07 files into the computer subdirectory that contains Epi Info, generally C:\EPI6  (separate instructions will be given for the web based data entry program when it becomes available).

6. Enter the audit data into the AUDIT07.REC file, by going to Epi Info's ENTER Program and typing in AUDIT07 when prompted for the name of the data (.REC) file.

7. [OPTIONAL] If a Local Option question was used, modify the report file to correctly display the results, if this has not already been done by your Area Diabetes Program.  It may be accomplished by making changes to the AUDIT07.RPT file.

8. Print out a summary report by entering Epi's ANALYSIS program, typing READ AUDIT07, pressing <F5> to send the report to your printer, and then typing RUN AUDIT07. [Note: if you have a networked printer or a newer USB printer, refer to alternate method of printing out the reports]

9. Supplemental Renal Preservation and Cardiovascular Disease Reports can be printed in a similar fashion, as well as a Means Report: begin at Epi's main menu, enter the ANALYSIS program, type READ AUDIT07, press <F5> to send the report to your printer, then type RUN RENAL07 or RUN CVD07 or RUN MEANS07.

10. Review the findings with your diabetes team and other interested persons.  Use the results to determine local strengths and weaknesses, and formulate a plan for improvement along with a timetable for its implementation.

11. Forward a copy of your data file (.REC file) to your Area diabetes consultant
