FY 2009 Audit Instructions

 

Quick Start Directions

For Manual Audits 

   

1. From a listing of all active diabetes patients, select in random fashion the appropriate number of charts to review
 
2. Review the 2009 audit form, definitions and criteria with all chart reviewers 
    
3.

Perform the chart audit and complete an audit form for each chart reviewed.   Be sure to complete all items.  
   
4. Access the WebAudit and enter the data from each audit form 
   
5. Clean and edit the data as necessary  
 
6. Print desired reports 
 
7. Once all data has been entered and cleaning/editing is complete, “lock” the data for the facility
