        Community Code Lookup  

 

	Find the code for a patient's community of residence by doing the following: 1. Click on the "Community Code Table" below. 2. Type in the name of the community to left of the Search button, then click on "Search" 3. The community code will be the 7 digit number in the first (leftmost) column.  It will be followed by the State, County, and Community name. 4. To return to the DMAUDIT.COM website, click on the "Go Back" arrow in the upper left corner of the window.
