| I don't know my facility's Area, SU or Facility codes -- where can I get them? | ||
| Actually, you
don't need them! The WebAudit will provide them for you
automatically. However, if for some reason you will be entering
your audit data via Epi Info or you are simply curious, you can look up your site's
2-digit Area, Service Unit, and Facility codes (referred to as an ASUFAC
code when combined to form 6 digits). Find them in the Audit
Codes
section. If your facility has never participated in a
diabetes audit before, your ASUFAC code may not be listed. If that
is the case, contact Dr. Ray Shields (see Support)
or call your Area Diabetes Consultant for assistance.
The Codes section also contains information on the 3-digit Tribal Affiliation Codes.
