The WebAudit will provide the correct name and identification codes for your facility automatically.  However, if for some reason you are curious, you can look up your site's 2-digit Area, Service Unit, and Facility codes (referred to as an ASUFAC code when combined to form 6 digits).  Find them in the Audit Codes section.   If your facility has never participated in a diabetes audit before, your ASUFAC code may not be listed.  If that is the case, contact Dr. Ray Shields (see Support) or call your Area Diabetes Consultant for assistance.
